The major duties of this position include (but are not limited to):
The clinical pharmacy specialist (CPS) is a licensed pharmacist with special training in clinical pharmacy, clinical pharmacokinetics, and clinical pharmacology.The CPS possesses the following background: graduate of an ACPE accredited school of pharmacy and possession of a Pharm.D. degree or equivalent experience and completion of an ASHP accredited pharmacy residency program or equivalent.The primary care CPS functions as a consultant to physicians and other health care professionals in their respective primary care clinics.
Preparation of medication histories for the patient's permanent medical record, other database, or both. Monitoring of drug therapy through direct patient-care involvement with clinical assessment and objective findings relating to patient's responses to drug therapy and communicating relevant findings and recommendations to the primary physicians or other health-care professionals, who are responsible for the patient's care. Assisting in management of medical emergencies, adverse drug reactions, and acute and chronic disease states when requested by the responsible staff physician. Provision of written consultations upon request in such areas as drug therapy selections, pharmacokinetics, parenteral nutrition support, and determination of therapy and points. Detection and reporting of adverse drug reactions in association with the responsible physician. Participation in drug-use evaluation and other quality assurance programs to maximize appropriate resource use and cost-effectiveness.Patient education and counseling.Participation in the education of health-care professionals.Supervises and trains pharmacy residents, graduate and undergraduate students as assigned. Assists or responsible for coordination, development and upgrading student training programs. Designs or participates in research studies involving drug therapy or clinical pharmacy functions.Assures compliance with FDA and DVA regulations pertaining to the use of investigational drugs.Manage an affective quality assurance program for the service. Responsible for drug-use control, such as the medication administration process and drug distribution in the patient-care area.Participates in committee functions relevant to clinical pharmacy participation.Incumbent schedules outpatient clinic appointment using Veterans Health Information Systems and Technology Architecture (VISTA) scheduling options.Uses established business rules to schedule appointments for veterans.This includes clinic visits, consultations and entries into the Electronic Wait List (EWL).Complies with the mandatory training requirements for the outpatient scheduling process.
QUALIFICATIONS REQUIRED:BASIC REQUIREMENTS
Education: Graduate of a degree program in pharmacy from an approved college or university.The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE), or prior to the establishment of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP).Verification of approved degree programs may be obtained from the American Council on Pharmaceutical Education,

Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the Unites States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.

English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
For the GS-12 Clinical Pharmacist:
Experience.Completion of the equivalent of 1 year of creditable experience at the next lower grade level which is directly related to the position to be filled.
Clinical Pharmacist/Pharmacy Specialist Knowledge, Skills and Abilities (KSAs).A Clinical Pharmacist spends a majority of time practicing in a major specialty (e.g., medicine, surgery, psychiatry, neurology, spinal cord injury, ambulatory care, rehabilitation medicine, geriatrics, medical research, etc.).Pharmacy Specialists are responsible for a major program area within Pharmacy Service (e.g., ADP systems, quality assurance/utilization review, drug information, etc.).In addition to the experience required in paragraph (a), pharmacists in these assignments must demonstrate the following knowledges, skills, abilities and other characteristics:
a.In depth knowledge of a specialized area of clinical pharmacy practice or a specialty area of pharmacy such as quality assurance/utilization review, ADP systems, drug information, etc.;
b.Advanced knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics;
c.Skill in monitoring and assessing the outcome of drug therapies including physical assessment and interpretation of laboratory and other diagnostic parameters;
d.Knowledge of the design, conduct, and interpretation of controlled clinical drug trials or other research related to health care;
e.Ability to communicate orally and in writing with a wide variety of individuals.This would typically include beneficiaries, professionals in other health care disciplines, and health related groups in the community; and
f. Knowledge of the standards related to distribution and control of scheduled and non-scheduled drugs (including research and investigational drugs.)This includes, but is not limited to, the standards established by DEA, FDA, VA, the State (Territory or District of Columbia, if appropriate), and JCAHO.
Quality of Experience: Experience is only creditable if it is post-licensure experience as a professional pharmacist directly related to the position to be filled.Qualifying experience must also be at a level comparable to pharmacy experience at the next lower level.Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist and subsequently passed the appropriate licensure examination.
Part-time Experience: Part-time experience as a professional pharmacist is credited according to its relationship to the full-time workweek.For example, a pharmacist employed 20 hours a week, or on a 1/2-time basis, would receive 1 full-time workweek of credit for each 2 weeks of service.
Fellowships: Fellowship programs are typically 2-year training programs in a specialized area of clinical practice.Applicants for these programs usually possess a Pharm. D. or an equivalent degree and have completed an accredited clinical or specialized residency (or both).Training as a fellow may substitute for creditable experience on a year-for-year basis.
Residency: A hospital residency recognized by the American Society of Hospital Pharmacists.It does not include internships which are shorter training programs designated to meet the requirements of boards of pharmacy of licensure.Hospital residencies may be substituted for experience on a year-to-year basis.
Professional Standards Board: The candidate tentatively selected will be peer reviewed by the Pharmacy Professional Standards Board and must meet Board requirements prior to appointment. This Board will evaluate the candidate qualifications for the position and determine the grade level. The incumbent selected will be appointed at the appropriate grade level based on (1) qualifying experience related to the work of the position, and (2) upon meeting physical requirements. Incumbent appointed below the GS-12 level maybe promoted up to this level after successfully meeting the experience and regulatory requirements and upon recommendation and approval by the Manager.
