JOB SUMMARY:

The Providence VA Medical Center provides high quality comprehensive outpatient   and inpatient healthcare to veterans residing in Rhode Island and southeastern Massachusetts. Each veteran patient is assured personalized care by a team of health care providers.

The Registered Nurse  Psychiatric Home Telehealth Care Coordinator  is responsible for managing care across the health care continuum for a select group of Veteran patients, assuring that care is timely, appropriate, of high quality and cost effective. He/she works closely with the mental health and primary care provider(s) and other health care professionals and team members, other clinics, internal and external services and community agencies. The incumbent provides professional assessment, coordination and planning of multiple health care services and acts on behalf of the veteran to assure that necessary clinical services are received and that progress is being made.  He/she provides ongoing evaluation of psychiatric care management services. He/she performs care management, a process that increases the likelihood that a patient will receive easily accessible, coordinated, continuous, high quality health care across all settings, including home. The incumbent uses evidence-based  and best-practices, technology, health education materials, as well as nursing and other clinical disciplines' expertise, to manage the health care process and limit the inappropriate use of health care services. He/she serves as backup for other Telehealth Care Coordinators both within their program line and/or within other program lines. The RN Psychiatric Home Telehealth Care Coordinator reports directly to the Program Director/Case Management and Telehealth.



QUALIFICATIONS REQUIRED:

Must be U.S. Citizen.  Graduate of a state approved, National League for Nursing Accrediting Commission (NLNAC)/Commission on Collegiate Nursing Education (CCNE) accredited school of nursing. A Baccalaureate degree in nursing with approximately two of three years of successful out-patient psychiatric nursing practice; or a master's degree in nursing and approximately one to two years of successful out-patient psychiatric nursing practice; or a doctoral  degree in nursing or a related field. Must possess an active, current, full and unrestricted professional nurse registration. A minimum of two to three years of successful experience as an out-patient psychiatric Case Manager National Certification as a Case Manager preferred. Prior medical surgical experience with psychiatry a plus.



You must be a U.S. citizen to qualify for this position.

After appointment, you will be subject to random testing for illegal drug use.

You will need to successfully complete a background security investigation before you can be appointed into this position.
